PARTICIPATION IN TREATMENT
You have the right to participate in the development of your care plan, including:
• To be involved in the service planning process
• to receive services with reasonable accommodation of individual needs and preferences, except where the health and safety of the direct care worker is at risk.
• Receive information in advance about the care, treatment, types of services, schedule or any changes in care, treatment, services, or schedule.
• Accept or refuse care or treatment.
• Formulate advance care directives, such as living wills or healthcare power of attorneys.
• Receive timely notice of anticipated discharge, or termination of care.
• To receive at least 10 calendar days, advance written notice of the intent of the home care agency to terminate services. Less than 10 days advance written notice may be provided in the event the consumer has failed to pay for services, despite notice, and the consumer is more than 14 days in arrears, or if the health and welfare of the direct care worker is at risk.
PARTICIPANT RESPONSIBILITIES
You share the responsibility for successful home care services by: •
Ensuring a safe and respectful environment for the client and home care staff.
• Participating in the plan of care, providing all necessary information, and reporting any significant changes or concerns to home care staff.
• Notifying home care staff if you will not be at home for a scheduled visit, except for emergencies.
• Providing the required supplies needed for your care.
